(Horizons Project)
As a grantee of the Ryan White HIV/AIDS Program, the Horizons Project provides care and prevention services for at-risk youth and those living with HIV or AIDS, ages 13-25. The clinic also provides care for perinatally HIV exposed infants and sexually assaulted patients of any age, including testing, treatment and counselling. Patient care at the clinic is often coordinated with one or more of the following health and wellness experts such as a dietician or social worker to help create a unique treatment plan for patients. The clinic is managed under leadership of Dr. Eric McGrath.
Our Mission
The Horizons Project provides comprehensive HIV prevention and care for ages 13-24 in a confidential, nonjudgmental, and youth-centered setting. We serve a diverse population and prepare youth for full transition to adult care. The clinic also offers pediatric HIV services; care for HIV exposed infants, children and their families, pediatric sexual assault support, and HIV Pre-exposure prophylaxis (PrEP), ensuring a continuum of care from birth through young adulthood.

- Ongoing medical care for persons living with HIV
- Support for perinatally exposed infants
- Sexual Assault STI testing & nPEP
- Primary Care & Routine Immunization
Case Management/Social Work
Horizons Project offers support with insurance applications & MIDAP coverage, transportation needs; housing options; GED, resume building, & interviewing skills; along with other community resources.
Mental Health Care
Horizons Project offers integrated, multi-disciplinary mental health care for our medical patients. Horizons Project offers two psychologists as well as a psychiatrist to support your mental care needs.
Outreach
Horizons Project works with numerous local & regional agencies to provide care including Avalon Healing Centers, Kids Talk Detroit, and Detroit Mercy Dental Center. Patients reaching the age of 25 years are fully transitioned to an appropriate adult HIV care center for their ongoing needs.

Tuesday: 9 am - 12 pm & 1 pm - 4pm
Thursday: 1 pm - 4 pm
No regular appointments on Wednesday & Friday, but staff available for refill requests and questions.
